Westlake Dermatology Rewards & Referral Program – Terms & Conditions Summary

By enrolling in the Westlake Dermatology Rewards Program (“Program”), you agree to the following terms:

Referral Rewards
Program members may share a referral link with friends or family. If a referred individual enrolls in the Program and completes their first appointment as a new patient, the referring member may receive reward points or credits in accordance with the Program rules.

Privacy & Patient Confidentiality
Referral rewards are issued automatically through the Program platform. Westlake Dermatology will not disclose the identity of referred individuals, appointment details, services rendered, or any medical information. Reward notifications will only confirm that referral points or credits were earned and will not identify the referred patient.

Consent to Referral Attribution
Individuals who enroll in the Program through a referral link acknowledge that the referring member may receive reward points or credits if the individual schedules and completes an appointment.

Program Eligibility & Limitations
Westlake Dermatology reserves the right to limit the number or value of referral rewards issued per member.

Redemption Restrictions

Reward credits may be redeemed only toward cosmetic products or services offered by Westlake Dermatology and may not be applied to services billed to insurance or federal healthcare programs (including Medicare, Medicaid, or other government-sponsored plans).

Program Modifications
Westlake Dermatology reserves the right to modify, suspend, or terminate the Program or its terms at any time.
